Discharge is the separation of mother from the hospital of birth either directly or on an early discharge program. Transfer includes transfer to a higher level facility for specialist care, transfer to a private or local facility for postnatal care and transfer to mothercraft hospitals. This does not include the early discharge program. Maternal death is the death of a woman during pregnancy, childbirth or within 42 days of birth, irrespective of the duration and the site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management.VIC: 'Separation Status – Mother' - Status at separation of mother (discharge/transfer/death). QLD: Was the mother discharged, transferred to another facility, died or remaining in after 28 days. Enter the day month and the year that the mother was discharged.SA: Mother's outcome for birth hospital/homebirth: discharged, transferred or died. The woman's final discharge date from birth hospital or hospital to which transferred for care. TAS: The status of the mother at separation: discharged, transferred or died. The mother was discharged from the hospital where the delivery, or immediate postnatal care took place. NT collect discharged (paper and online) transferred (paper) and died (paper). NT (paper) also include left against medical advice. NSW, QLD, SA and NT (paper) collect mode of separation/place of transfer as a free text field. QLD also collect early discharge. WA PDC collects this via linkage to Hospital Morbidity Data System.
